Long Term Condition Management

Appello is a unique, subscription based, nurse-led telehealth service offering a complete home-based monitoring, advice and support service for individuals with a long term health condition, initially monitoring Diabetes, Asthma, Coronary Heart Disease (CHD) and Chronic Obstructive Pulmonary Disease (COPD).

The service will:

• collect and monitor an individual’s vital signs appropriate to their long term condition
• provide electronic storage of the monitored readings in the individual's personal health record
• transmit the readings for review to the Appello monitoring centre
• alert where an individual either fails to take a reading or the reading is outside the specified range
• forward any adverse readings as an alert to a qualified nurse advisor or other health professional
• provide a process for telecarers or nurse advisors to contact the individual and progress the management of their health care including advising them what next steps to take.
